Who do I contact to change my name and address for Medicare purposes?

Keeping your personal information up to date with Medicare is more than just a clerical necessity; it ensures you receive your Medicare card, benefit statements, and the annual “Medicare & You” handbook without interruption. However, many people are surprised to learn that Medicare does not update this information directly.

 

Because Medicare is an entitlement program administered through federal records, you must contact the specific agency responsible for your enrollment record—usually the Social Security Administration (SSA) or the Railroad Retirement Board (RRB).

 


The Primary Contact: Social Security Administration (SSA)

For the vast majority of beneficiaries, the Social Security Administration maintains the master record for Medicare. Even if you are not yet receiving monthly Social Security retirement benefits, the SSA handles the administrative side of your Medicare enrollment.

 

1. How to Change Your Address

Changing your address is a straightforward process that can often be completed without visiting an office.

  • Online (Fastest): Log into your personal my Social Security account. Once logged in, navigate to the “My Profile” tab to update your mailing address. This change will automatically flow through to Medicare’s systems.

     

  • By Phone: Call the SSA at 1-800-772-1213 (TTY 1-800-325-0778). Representatives are available Monday through Friday, 8:00 a.m. to 7:00 p.m. local time.

     

  • In Person: You can visit your local Social Security office. It is highly recommended to use the SSA Office Locator to find the nearest branch and check if an appointment is required.

     

2. How to Change Your Name

A name change is more intensive than an address update because it requires legal proof. You cannot change your name online.

  • Submit Form SS-5: You must complete the Application for a Social Security Card (Form SS-5).

     

  • Provide Evidence: You will need to provide original documents (or certified copies) proving your legal name change, such as a marriage certificate, divorce decree, or court order.

  • Identity & Citizenship: You may also need to provide proof of identity and, in some cases, U.S. citizenship.

     


For Railroad Employees: Railroad Retirement Board (RRB)

If you receive Medicare benefits as a retired railroad worker or as a spouse of one, your records are maintained by the Railroad Retirement Board (RRB) rather than the SSA.

 

  • Online: Use the myRRB portal on the official RRB website.

     

  • By Phone: Call the RRB toll-free at 1-877-772-5772.

     

  • In Person: Contact your local RRB field office for assistance with name or address modifications.


Why You Must Notify Your Private Plans Separately

If you only have Original Medicare (Part A and Part B), updating Social Security is sufficient. However, if you are enrolled in private Medicare coverage, you have an extra step.

Important Note: Social Security does not notify private insurance companies of your move.

You must contact your plan provider directly if you have:

  1. Medicare Advantage (Part C)

  2. Medicare Part D (Prescription Drug Plans)

  3. Medicare Supplement (Medigap)

Moving to a new zip code or state often triggers a Special Enrollment Period (SEP), allowing you to switch plans if your current provider does not operate in your new area. Failure to notify them could result in a loss of coverage or claims being denied.
